The effect of pyloromyotomy on serum and luminal gastrin in infants with hypertrophic pyloric stenosis.

چکیده
Previous studies of the pathogenesis of congenital hypertrophic pyloric stenosis (CHPS) have implicated immunoreactive gastrin, although no consistent relationship has been demonstrated. In this study we have examined the effect which pyloromyotomy has on serum and luminal gastrin concentration after a mechanical and protein stimulus. Seventeen infants were examined preoperatively, and 1 week after pyloromyotomy. On each occasion, samples of serum and gastric contents were collected from fasting infants. Sixty cubic centimeters of water was placed into the stomach and further samples collected 20 min later. The water was then aspirated and replaced by 60 cc of 10% peptone broth and a third set of samples collected after 20 min. All samples from each patient were analyzed for immunoreactive gastrin in the same assay. Pyloromyotomy did not alter fasting serum gastrin (119.3 pg 2 11.9 preop vs 164.7 f 29.9 postop) nor did it alter the gastrin response to water. Pyloromyotomy decreased the incremental serum gastrin response to peptone broth (66.6 + 16.9 preop vs 18.9 + 11.7 postop). Luminal gastrin concentration was not significantly affected by pyloromyotomy. When the preand postoperative serum gastrin increments for water and peptone were plotted against the fasting gastrin levels, an inverse relationship was apparent which was statistically significant by regression analysis. Seen in this way, intragastric water and peptone have a dual effect on serum gastrin; a rise if the fasting serum gastrin concentration is low; a fall or lesser rise if the fasting serum gastrin concentration is high. The data suggest that the direction and magnitude of serum gastrin response to intragastric water or peptone is set by the fasting level, and is independent of pyloromyotomy.

Increased serum immunoreactive gastrin levels in idiopathic hypertrophic pyloric stenosis.
The serum immunoreactive gastrin (IRG) level in infants with confirmed idiopathic hypertrophic pyloric stenosis (IHPS) has been determined and compared to that found in vomiting infants without IHPS, in normal infants, and in normal adults. The mean serum IRG level of normal infants (103 +/- 9 pg/ml (mean +/- SEM) exceeded that of normal adults (28 +/- 5 pg/ml). The preoperative mean serum IRG ...

Background & Aims: In hypertrophic pyloric stenosis (HPS) the hypertrophied muscles of pylorus cause partial gastric outlet obstruction. The standard management of HPS is pyloromyotomy. However, successful treatment of HPS with atropine sulfate also has been reported. We aim to compare the outcomes of medical and surgical treatment for HPS. Materials & Methods: In this comparative clinical tri...
متن کاملHypertrophic Pyloric Stenosis in a Premature Infant: A Case Report
Hypertrophic pyloric stenosis is uncommon in premature babies. In term infants, the typical presentations include a palpable mass over the epigastric area, exaggerated gastric peristalsis after feeding and projectile non-bilious vomiting. However these symptoms are not commonly seen in premature infants. The onset of symptoms in pyloric stenosis in premature infants is later than that in term i...
متن کاملUse of Ultrasound Measurements to Direct Laparoscopic Pyloromyotomy in Infants
BACKGROUND Laparoscopic pyloromyotomy is associated with an increased risk of incomplete myotomy compared with open myotomy. We hypothesized that utilizing ultrasound measured length to direct laparoscopic pyloromyotomy would reduce the risk of incomplete pyloromyotomy without a concomitant increase in the risk of mucosal perforation. METHODS Infants (n=43) with hypertrophic pyloric stenosis ...
متن کاملVomiting after pyloromyotomy for infantile hypertrophic pyloric stenosis.
An analysis of the factors which may predispose towards postoperative vomiting after pyloromyotomy for hypertrophic pyloric stenosis was carried out in 72 infants at this hospital. 26 (36%) infants experienced moderate to severe postoperative vomiting of sufficient intensity to cause the postoperative feeding regimen to be modified or interrupted. Only two parameters were found to be of statist...
